The Benefits of FEESĀ
Both FEES and the videofluoroscopic swallow study (VFSS/MBSS) are valid instrumental assessments, and each has strengths. In skilled nursing, rehabilitation, and long-term care settings, FEES offers practical and clinical advantages that matter for this population specifically.

No Patient Transport
The evaluation happens in the patient's room. No ambulance, no staff escort, no lost day, no transport costs, and no exposure risk for a medically fragile resident.
For patients who are difficult to transport such as ventilator-dependent, bariatric, behaviorally complex, or simply unwilling to leave the building, a bedside FEES assessment may be the only realistic path to an instrumental study.

No Radiation Exposure
FEES involves no ionizing radiation, so it can be repeated as often as clinically indicated. This matters for patients whose status is changing, who are progressing through diet advancement, or who need documentation of decline over time. Serial studies carry no cumulative exposure.

Real Food in Real Environment
Patients are evaluated using actual food and liquids of varying consistencies instead of barium which provides a closer representation of their daily swallow function without barium interference.

Direct Visualization of Anatomy
FEES provides a direct view of the pharynx and larynx rather than a radiographic shadow. Vocal fold mobility, tissue changes, and laryngeal lesions are visible. When we observe something outside the swallowing question that appears clinically significant, we note it and recommend follow-up.

Reduced Time Constraints
Because there is no radiation exposure limiting study duration, the scope can remain in place for a more extended period of time. Fatigue-related decline is a common contributor to aspiration and is easily missed on a brief study.Ā A patient who appears safe on the first three swallows may not on the twentieth.

Tracheostomy & Ventilator Patients
FEES can be performed without disconnecting a patient from ventilator support, and without the positioning demands of fluoroscopy. This population also tends to require repeat assessment, where the absence of radiation exposure is a direct benefit.

Low Risk & Well Tolerated
Complication rates across large published series are consistently under 1%, with the most common findings being transient discomfort and occasional gagging.
